I wanted to get the Angel of Mercy Wings rn. But I need to do about 800+ more surgeries.

Is it counted if doing surgery to Surg-E ?
Is it counted if doing surgery without using Blingin' set?

1. Pretty sure no, since you need vicious malady too.
2. You must have the full blingin set to do this, or it wont count

You must do 2k (two thousand) surgeries, 2000. But your past surgeries count! If you have already done 2000 surgeries, buy the full blingin set and do a vicious malady. So basically you have to do 2000 surgeries. However, in order to get the Mercy wings you must have done 2000 surgeries already, and completed a vicious with the FULL blingin set on.

Yes, doing surgery without the set counts (as long as you've done these after the Blocktor's Orders update). You will only need it when you completed 2,000 surgeries and will cure a vicious malady - giving you the Angel of Mercy's Wings.

Train-E can't be counted to quests, so no.
