No, but seriously. I have this problem all the time. Just do the stuff that you haven't done yet 1 step at a time. Don't rush yourself, just know by doing it you'll get it done eventually.

you're not alone with this one

If you can't seem to focus on something that needs to get done, just go outside for a run or for a bike ride (I can't run lol).

I would suggest making a checklist for the things you need to do. For example, you've got a poem to present on Wednesday, a quiz in history on Friday, a test in math on Friday too, and the regular amount of ~2 hours worth of homework every night. Since the quiz in history is way too easy, you can allot time to study for that under Thursday. However, you have a very, very hard math test that you can't grasp the concepts of yet, so you should allocate at least 30 minutes a day working out practice problems, more if necessary, and maybe 2 hours the night before the test. For the poem presentation, you might as well recite it for 20 minutes a day until you got it memorized. Your checklist should look like this:
    Monday:
    ☐ Finish homework due Tuesday
☐ Spend 30 minutes studying for the math test
☐ Recite poem for 20 minutes
    Tuesday:
    ☐ Finish homework due Wednesday
☐ Spend 30 minutes studying for the math test
☐ Recite the poem to mom, get feedback, recite until memorized
    Wednesday:
    ☐ Finish homework due Thursday
☐ Spend 30 minutes studying for the math test
☐ Spend 30 minutes studying for the history quiz
    Thursday:
    ☐ Finish homework due Friday
☐ Spend 2 hours studying for the math test
☐ Spend 1 hour studying for the history quiz

Make sure you're allotting time that you know is more than enough. Put your phone into a different room, turn off all instant messaging and close all your games, (I would suggest using BlockSite, which is a chrome extension, and you should block off all the social media/forums websites).

Note: Make sure you make the checkbox on paper. Check out this study: here.

People procrastinate at some point. Heck, it can even happen to some of the best people, so don't sweat it.
Just leave a mental note (a really huge one!) to ensure that it doesn't go too far.

You'll eventually grow out of it, but it'd be awesome if you do that sooner.
If you don't feel like -for example- doing your homework: Go out for a walk, or maybe do some meditating on a beautiful day, do that and you'll definitely get back on track.

Well uh, I had the same issue myself self and I know how frustrating it is I suggest you take down notes of everything that you have to do and prioritize them on your list. Make categories and put things in their own categories. Score them out one by one every day.

Let it all out on paper. When I write things down on a paper, the scale diminishes. Try to focus on what's important, make sure you're concentrated on the work you do. And yes, feeling guilty and frustrated is normal after you realize that procrastinating your work is the worst thing you could have done.
